The Division is required to make an approval or denial decision no later than May 27, 1997, therefore, your response to the following adequacy review concerns should be submitted to the Division by May 20, 1997. If you feel more time is needed to complete your reply, an extension to the decision date can be granted by the Division. This will be done upon receipt of a written waiver of your right to a decision by May 27, 1997 and request for additional time. This must be received no later than May 23, 1997. i. Pursuant to Rule 6.4.3, some additional information is required on the Exhibit C map showing the topography of the Deer Creek Quarry (scale 1" = 100'). Please show on the map the immediately adjacent landowners and show the location of the power line and any other permanent man-made structures contained within the area of the affected land or within 200' of the affected land. Also list the name of the owners of each structure on the map. Show on this map the total area to be affected. 2. In Exhibit D, describe in more detail how this quarry will be mined. On an Exhibit C map (or maps) show the location of processing facilities, product and topsoil stockpile locations, equipment storage areas, and the currently approved sediment ponds, and ditches.
