The Division is required to make an approval <br />or denial decision no later than May 27, 1997, therefore, your <br />response to the following adequacy review concerns should be <br />submitted to the Division by May 20, 1997. If you feel more time <br />is needed to complete your reply, an extension to the decision date <br />can be granted by the Division. This will be done upon receipt of <br />a written waiver of your right to a decision by May 27, 1997 and <br />request for additional time. This must be received no later than <br />May 23, 1997. <br />i. Pursuant to Rule 6.4.3, some additional information is <br />required on the Exhibit C map showing the topography of the <br />Deer Creek Quarry (scale 1" = 100'). Please show on the map <br />the immediately adjacent landowners and show the location of <br />the power line and any other permanent man-made structures <br />contained within the area of the affected land or within 200' <br />of the affected land. Also list the name of the owners of <br />each structure on the map. Show on this map the total area to <br />be affected. <br />2. In Exhibit D, describe in more detail how this quarry will be <br />mined. On an Exhibit C map (or maps) show the location of <br />processing facilities, product and topsoil stockpile <br />locations, equipment storage areas, and the currently approved <br />sediment ponds, and ditches. <br /> <br />
