PART I Pale No. 12 Permit No. COG-850000 D. GENERAL MONITORING, SAMPLING AND REPORTING REQUIItEMENTS 1. Routine Reportine of Data Reporting of the data gathered in wmpliance with Pan I.B.3. shall be on a ouarterly basis. Reporting of all data gathered shall wmply with the requiremenu of Pan I.D. (General Requiremenu). Moni[oring results shall be summarized for each calendar quaver and reported on Division approved dischazge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the agencies listed below so [hey are ietxived no later than the 28th day of the mon[h following the end of the quaner (for example, the DMR for the first calendar q»aner must be received by the Division by April 28th). If no dischazge occurs during the reporting period, "No Dischazge' shall be reported. The DMR forms wnsist of four page -the top "original" espy, and three anached no-carbon-required copies. Afrer [he DMR form has been filled out and signed, the four espies must be sepazated and distributed as follows: The first original signed espy of each dischazge monitoring report (DMR) shall be submitted to the Division at the following address: Colorado Depanmea[ of Public Heal[h and Environment WQCD-PE-B2 a300 Cherry Creek Drive South Denver, CO 80222-1530 The US Environmental Protection Agency Region VIII no longer requires submittal of duplicate copies of the DMR to [heir office, [hcrefore, the sewnd, third and fourth espies aze for the permittee rewrds. The Dischazgc Monitoring Report forms shall be filled out accurately and wmpletely in accordance with requiremenu of this permit and the instructions on the forms. They shall be signed by as authorized person as identified in Pan I.D.7. Calculations for al! limitations which require the averaging of mea_uremencs shall utilize an arithmetic mean unless otherwise specified by [he Division in the permit. 2. ReportinE - Stormwater Annual Report The permittee will be requved to submit an Annual Report, wvering January 1 through December 31 of each yeaz, on the overall compliance with the SWMP. Tne Annual Report will wntain, at a minimum: a) Name of permittee, address, phone number, and permit certification number. b) A report on the facility's overall compliance with the SWMP. c) A summary of each wmprehensive stonnwater facility inspection made, including date, findings, and action taken. d) Resul[s and interpretation of any stormwater monitoring performed. e) The report shall be signed and certified for accuracy by the perminee, including the certification language wntained in Pan LD.7. of the permi[. The Annua! Report will be due to the Division on or before February 15 of each yeaz, after the first full yeaz of coverage under the permit. The enact due date for the permirtee's first Annual Report will be listed in their permit cenifica[ion. The Division reserves the right to require addi[iona] information in the report, on a case-by{ase basis, as needed. All repons required for submittal shall be signed and certified for awuraty by the permittee (see Part I.D.7.).
