GENERAL MONITORING, SAMPLING, AND REPORTING REQUIREMENTS Routine Reoortine of Data Reporting of the data gathered in compliance with Part I.B.3. shall be on a uaq nerly basis. Reporting of all data gathered shall comp with the requirements of Part I.D. (General Requirements). Monitoring results shall be summarized for each calendaz quarter and reported on Division approved discharge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the agencies listed below so they are received no later than the 28th day of the month following the end of the quarter (for example, the DMR for the fast calendar quarter must be received by the Division by Apri128th). If no discharge occws during the reporting period, "No Discharge" shall be reported. The DMR forms consist of fow pages -the top "original" copy, and three attached no-carbon-requued copies. The Dischazge Monitoring Report forms shall be filled out accwately and completely in accordance with requirements of this permit and the instmctions on the forms. An authorized person as identified in Part I.D.7 shall sign them. Calculations for a111imitations, which require the averaging of measwements, shall utilize an arithmetic mean unless otherwise specified by the Division in the permit. 2. Renortine - Stormwater Annual Resort The perrnittee will be required to submit an Annual Report, covering January 1 through December 3] of each yeaz, on the ove~ compliance with the SWMP. The Annual Report will contain, at a minimum: a) Name of perminee, address, phone number, and permit certification number. b) A report on the facility's overall corr>pliance with the SWMP. c) A summary of each comprehensive stormwater facility inspection trade, including date, findings, and action taken. d) Results and interpretation of any stormwater monitoring performed. e) The report shall be signed and certified for accwacy by the pertninee, including the certification language contained in Part I.D.7, of the permit. The Annual Report will be due to the Division on or before Febmary 15 of each yeaz, after the fast full yeaz of coverage under the pemtit The exact due date for the pemtittee's fast Annual Report will be listed in their permit certification. The Division reserves the right to require additional information in Ibe report, on a case-by-case basis, as needed. All reports required for submittal shall be signed and certified for accwacy by the permittee (sec Part LD.7.). Representative Sampline Samples and measwements taken as required herein shall be representative of the volume and nature of the monitored discharge. All samples shall be taken at the monitoring points specified in this permit and, unless otherwise specified, before the effluent joins or is diluted by any other wastestream, body of water, or substance. Monitoring points shall not be changed without notification to a: approval by the Division. Ifthe permittee monitors at the point of discharge any pollutant limited by the permit more frequently than required by the permit, using approved test procedwes or as specified in the permit, the result of this monitoring shall be included in the calculation and reporting of data to the Division.
