Permit <br /> Number 21447 <br />HGDAL4 Attachment 7-4 to requirement missed in TR-07 <br /> Tab 7, PAP <br />Shallow Well See footnote 2. permanent permanent well <br />No. 1 well <br />water supply See footnote <br />well 3. <br />A statement on page 5 of Tab 13 of the PAP states that wells HGDALI and HGDAL2 were <br />abandoned in 2005 in accordance to the rules and regulations of the Colorado Division of Water <br />Resources. Monitoring was terminated, but the wells have not been sealed and no abandonment <br />reports have been filed. <br />Z No completion report was found in the PAP, but one was obtained from CDWR. A copy is <br />enclosed with this inspection report. <br />s According to a letter in the files of the operator, Well 22772F (see Exhibit 7-1, PAP) was <br />abandoned and replaced with well 24576F. There is no abandonment report in the PAP, but a <br />well abandonment affidavit was acquired from CDWR and a copy is enclosure with this <br />inspection report. <br />3.5. Are there any impoundments on site? <br />There aze two sedimentation ponds. They aze the the Truck Loop Pond and the Rail Loop Pond. <br />Both impoundments are earthen embankments. The Truck Loop Pond has a corrugated metal <br />pipe primary spillway and a corrugated metal pipe emergency spillway. The Rail Loop Pond has <br />a combined, concrete-lined, primary and emergency spillway. These impoundments aze permitted <br />as temporary impoundments (PAP, Tab 12, pages 2 and 3). As-bunts are at Exhibits 12-3 and 12- <br />4 to Tab 12, PAP). There are maintenance task line items in the reclamation cost estimate for <br />these impoundments (task 040). Reclamation cost estimates of the Ponds are at tasks 018, 019, <br />046 and portions of other topsoil and seeding tasks. <br />3.6. Are there stock tanks or dozer basins on site? If so, what is their average size and aerial <br />density? Compare with cost estimate. <br />There are no stock tanks or dozer basins on the site. <br />3.7. Are there any diversions on the site? <br />There are nine ditches internal to the disturbed azea. None divert natural drainages. See Table 12- <br />1 on page 7 (OS/OS) and Exhibit 12-2, 3/2007, of the PAP for all but Ditch 5. See (electronic <br />historical file) Exhibit 12-2, 6/28/2000, for the location of Ditch 5. Roadside ditches are not <br />identified herein. All ditches and diversions are documented in the permit. All ditches aze to be <br />reclaimed (see Task 020) in the general grading of the disturbed azeas. <br />Part d -Structural Facilities -Completed at Permit Renewal and Midterm <br />=1.1.
