(1) The well pressure, temperature, and flow rate instrumentation will <br />be checked for accwacy; <br />(2) The well valves will be checked for possible leakage; and <br />If steps 1 and 2 rectify the problem, normal operation of the well shall <br />continue; but if steps 1 and 2 do not rectify the problem the following <br />steps shall occur: <br />(3) Injection into the solution mining well shall be discontinued and <br />the density of the fluids in the well shall be balanced. Verbal <br />notification shall be given to the EPA within twenty four (24) <br />hours and written notification within five (5) days. <br />(4) A mechanical integrity test of the casing shall be performed by <br />pressurizing the annulaz space between the casing and tubing with <br />nitrogen gas. The nitrogen gas presswe shall correspond to the <br />hydrostatic pressure of the top of the cavity. A mechanical <br />integrity test of the tubing shall be performed by closing the <br />wellhead tubing valve and measuring the pressure of the tubing; <br />(5} A temperatwe log shall be performed to determine if there is any <br />significant temperatwe change above the nahcolite-bearing zone; <br />(6) The results of these procedures shall be evaluated to determine the <br />best solution for correcting the problem. The use of additional <br />logging techniques maybe required to accurately pinpoint any <br />mechanical failures; and <br />(7) Specific corrective-action procedwes shall be developed and <br />implemented with the assistance of the appropriate agencies. <br />*** <br />Al] other provisions and conditions of Final Permit No. C030858-00000 shall remain as <br />originally issued, or as previously modified. <br />Date t Stephen S.
