Where anon! inspection are impracticable, <br />certifiation is required once every three years by a Registered Professional Engineer that the site is in compliance <br />with the plan. These inspection must be documented and ~,mTer;r..l in the compliance report (see Part I.D.7.c <br />of the permiQ. <br />RecortHceeoirtQ and Revortirta <br />e. Rtxordkeeping and Internal Reporting Procedttres. Incidcata such as spills or other discbarga shall be <br />included in the records. Inspxtion and maintenance activities shall be documented cad recorded. This does not <br />relieve dte permittee of the tesponibility of reporting spills as required under section 25-E~01(2) of the Colorado <br />Water Quality Control Act. <br />b. Annual Report for Alive hfining Opaatirstu. The permittee will be required to submit an annual report, <br />covering January 1 through December 31 of each year, on the compliance with the SWMP. The annual report <br />will contain, at a minimum: <br />1) A report ov the facility's overall compliance with the SWMP. <br />2) Name of permittee, address, phone number, and permit artifiutiov number. <br />3) A summary of each wmprehenive stormwater Gcility inspection made, including date, findings, and utiov <br />taken. <br />4) Results and interpretation of any stormwater monitoring performed. <br />The annual report will be due to the Division oa or before February 15 of euh year. The enact due date for the <br />pertvittee's first annual report will be listed in their permit certifiutiov. in no case will oo anon! report be due <br />before February 15, 1994, unless specifically directed by the Division. The Division reserves the right to roquire <br />additional information in the report, on a case-by~ase basis, as needed. <br />All reporu required for submittal shall be signed and certified for accuracy by the permittee. (See Part I. D.9 of <br />the permit.) <br />
