.., ~ • <br />8) Pages 4-46 and 47 include a discussion of plans for replacing topsoil and testing <br />substitute topsoil material where topsoil was not available for redistribution. These <br />pages should be updated to explain where topsoil was redistributed and to what <br />depth(s). Results of lab analyses of topsoil substitute material, and the location of <br />those samples should also be included in the permit. <br />9) Page 4-71b lists the No. 1 Mine Portal area as an area that will not be reclaimed. <br />Rule 4.14.2 requires the return of all disturbed areas their approximate original <br />contour and elimination of face-up areas for underground mines. Please revise this <br />page accordingly. <br />,/10) On pages 4-80, 4-95 and 4-99, or other appropriate locations}, a discussion of the <br />fate of the loadout (conversion to a housing development) should be included. <br />,/ 11) On page 4-85, the tense should be changed to clarify that this work has been done. <br />J 12) Mention should be made, somewhere in the text, of the new plumbing, approved in <br />TR-18, which combines the No. 1 and No. 3 Mine discharges. Although this was <br />addressed in an appendix in the TR, mention should be included in the text for <br />clarity. <br />13) The reclamation cost estimate was updated to reflect 1995 costs for labor and <br />equipment. The reclamation cost estimate totals $162,444.00. The bond held by the <br />Division is in the amount of $207,733.00. The estimate was also revised to include <br />a cost of $10,000 for pond cleaning. A copy of the revised estimate is included for <br />your review. Please revise the reclamation cost estimate in Section 4.4.2.4 <br />accordingly. <br />14) Page 9 of Appendix 3-W contains a reference to the old diversity standard on page <br />4-64. Page 4-64 now includes the currently approved diversity standard. Page 9 of <br />the appendix should be revised. <br />15) Although use of an extended reference area in the mountain shrub community <br />adjacent to the mine was approved in TR-14, there is no map in the permit showing <br />the location and extent of the extended area. Please include a map indicating the <br />extent of the extended reference area. <br />16) Please revise the Probable Hydrologic Consequences section of the permit to account <br />for the additional discharge from the No. 3 Mine. <br />
