low-flow surface water elevations. Please cleazly label the current thalweg and the 1996 <br />thalweg elevations on the cross-sections. Your plan shall also include a project overview <br />with the locations of the mining activities cleazly labeled. The proposed mining plan shall be <br />submitted to this office for approval no later than 30 days prior to the commencement of <br />work. You shall not commence work until you receive written verification from this office. <br />5) The surveys required by Special Condition No. 3 of your permits shall include <br />longitudinal data of each transect, estimated river mile location, and east/west coordinates. <br />In addition, you shall include a table of the surveyed elevations and related distance from the <br />established monument. The surveys should be performed regardless of the status of mining. <br />6) Within 60 days from the completion of the mining operation, you shall submit a <br />mining report which should summazize the mining activity within the last yeaz, including a <br />profile and cross-sectional perspective of the location of mining, bank stability analysis, post- <br />mining survey with annotated observations at time of survey, the quantity of material mined, <br />schedule of work, and photographs of all impacted azeas, including bank stability of <br />upstream and downstream areas. <br />7) To reduce operational errors during excavation, asurvey-type leveling device shall <br />be installed on all excavating equipment and shall be used at all times during excavation to <br />ensure that the equipment operators have the capability to adequately determine excavation <br />depths. <br />8) You shall provide an overview plan showing the location of the low flow main <br />channel, banks, survey transects, and monuments. <br />9) You shall develop a plan for any necessary protection for streambed or <br />streambank stability within the project vicinity, including upstream and downstream <br />neighboring properties. <br />10) As part of your 401 water certification as required under Section 404 of the <br />Clean Water Act, you shall meet the parameters and practical quantification levels ("PQL") <br />as established by the Water Quality Control Division of the Colorado Deparhnent of Public <br />Health and Environment and as outlined in the enclosed table. Prior to the commencement <br />of work, you shall coordinate with the State to develop a plan for the timing and frequency <br />of monitoring within the Animas River. <br />11) In order to minimize any potential indirect impacts to the Animas River system, <br />the mining area must be regraded after the completion of mining activities and must be <br />completed prior to April 1. Regrading includes filling in depressions, grading the <br />construction excavation site to a minimum of 2 per cent, and removing all temporary fills <br />from the river bed and banks. <br />
