<br />RIDER TO MINING PERMIT <br />Permit Date: July 6, 1996 <br />Permit No: C-80-004 <br />The following additional stipulations form an integral part of the foregoing permit: <br />Stipulation No. <br />Description <br />22. The permittee will submit additional bond in the amount of <br />$25,147.00 within 30 days after public comment period ends. Permit <br />will not issue prior to bond acceptance. <br />23. Within 30 days of permit issuance, the operator will submit a <br />relevant showing statement in lieu of certification for the McClane <br />access/haul road, pursuant to Rule 4.03.1(1)(d)(ii) <br />24. Table 3.1-1 will be expanded to address the reclaimed channels in the <br />vicinity of the reclaimed lower access road and sediment pond/topsoil <br />stockpile site. Riprap sizing, gradation, and installation design <br />details will be provided for those channel segments which require <br />riprap protection, and reclamation plan text will be amended to <br />specify that concrete from foundations and other sources will be <br />buried at least four feet below grade, in locations other than the <br />reclaimed channels. These amendments will be submitted within a <br />complete technical revision application within 60 days of permit <br />issuance. <br />25. A narrative description, longitudinal profile, and appropriate cross <br />sections of the temporary diversion channel created by the road <br />crossing of the McClane drainage channel will be submitted for <br />inclusion in the application, along with applicable flow parameters. <br />Sufficient information will be submitted to demonstrate compliance <br />with the requirements of 4.05.3(1), (2), (4), (5), (6) and (7), and <br />4.03.1(4)(a)(i). These amendments will be submitted within a <br />complete technical revision application within 60 days of permit <br />issuance. If compliance with the cited requirements cannot be <br />demonstrated, the revision application will address necessary <br />modifications to the diversion channel. <br />
