below ground surface with nitrified cement. A cap of 25 sacks Class G with quick set <br />additives was pumped into the top o1'the wellbore/casing annulus to contain any release <br />of nitrogen from the circulated cement. The drilling and completion report indicated that <br />the nitrified cement did reach the surface prior to the placement of the cement cap; <br />A CAST/CBL log was run and analyzed by Halliburton. The analysis by qualified log <br />analyst indicated good to excellent cement from TD to 1160 feet BGS (above the A- <br />Groove; poor cement from 1160 feet. to 870 feet; good cement from 870 to 705 feet; and <br />poor cement to the top of the log at fi00 feet. The log was only run from TD to 600 feet <br />because that was the fluid level in the well and the casing must be fluid filled for the <br />logging tool to function properly. Based on the cementing records and the CBL logs, the <br />cement is considered adequate to isolate the USDWs at this location; <br />A descriptive schematic diagram ofthe injectionwell as-constructed and formation tops <br />was submitted to the Director on August 6, 2002; and <br />4. A copy of the MIT pressure test to demonstrate Part I of mechanical integrity and the <br />results of a temperature cool down log to demonstrate Part II of mechanical integrity was <br />submitted on June 4, 2002. <br />All required information has been submitted, and has been reviewed and approved by the <br />EPA. WRNM has complied with al] pertinent conditions of UIC Permit No. CO3358-00000, <br />Part II Section C. 2. "Prior to Commencing Injections. Therefore, effective upon your receipt <br />of this letter, Administrative approval hereby is Eranted for injection into the SH under the <br />conditions of UIC Area Permit CO3358-00000. <br />As of this approval, responsibility for Permit compliance and enforcement is transferred <br />to the Region VIII UIC Technical Enforcement Program office.
