Mid-Term Review <br />Eastside Mine <br />I. Introduction <br />This document presents the results of the Division's mid-term review of the <br />Eastside Mine. This review was conducted to fulfill the requirements of Rules <br />2.06.2, 3, 5, 7 and 2.08.3. Rule 2.OA.3 requires that the Division conduct a <br />review of each permit issued, prior to its mid-term. As the permit was issued <br />for 5 years, the mid-term is at 2 1/2 years. Based on the review, for good <br />cause shown, the Division may require reasonable revision or modification of <br />the permit provisions to ensure compliance with the Act and Regulations. <br />Rules 2.06.2, 3, 5 and 7 require that experimental practices, mountain top <br />removal, variances from AOC, and variances from contemporaneous reclamation be <br />reviewed by the Division. The Eastside Mine does not have any operations <br />under these categories. <br />Rule 3.02.2(4) requires that the Division review the amount of bond and the <br />terms of acceptance at the mid-term. The bond will be reviewed at a later <br />date with the results forwarded to Eastside. <br />The mid-term review consisted of a detailed review of the Eastside Mine <br />permit. The Division also reviewed subsequent revisions and stipulation <br />responses to insure that all permit commitments and conditions were being <br />followed. Hydralogic monitoring data was reviewed to assess the discussion of <br />hydrologic impacts. Specific issues of hydrologic monitoring are contained <br />under separate cover discussing the 1986 Annual Hydrologic Report. <br />II. General <br />1. The permit application is outgrowing the one volume. The permit <br />application should be revised to a two volume format. The Division <br />is now only requesting 2 copies of all material, except in cases <br />where extensive review is needed. For the mid-term, only 2 copies <br />will be needed. The permit application would be easier to reference <br />with Appendix Tab Sheets and a table of contents that references the <br />page number of major sections, such as Vegetation, Hydrology, <br />Operations Plan, etc. <br />2. The "Application Forms for a Permit" must he updated to reflect <br />current conditions for the mine company and site. <br />3. Identification of Interests - the names and addresses of officer and <br />principles must be reviewed for any needed revisions. Also, is the <br />Resident Agent still the same? <br />4. Table 1 must be updated to include numbers of permits received. <br />
