Presently, the <br /> currently approved permit presents a table of facilities and very minimal discussion. Each <br /> building, structure, utility corridor, should be described and referenced by the same name <br /> as is shown on the map. Structures should be clearly identified as existing at the time of <br /> the original permit issuance and those which were constructed after the permit was issued. <br /> 9. Rule 2.05.3(3)(c) requires descriptions, of roads, conveyors, and rail systems. Presently, <br /> this section of the permit describes only roads. Please add the appropriate information for <br /> the conveyors and rail system at this site. <br /> 10. Text under Rule 2.05.3(4) indicates the area north of ditches DI I and D19 was topsoiled, <br /> mulched, and seeded in September of 1990, but Map 11 and the 1995 Annual Reclamation <br /> Report map indicate 1991. Correct this information as appropriate. <br /> 11. On page 2.05-9, Map 133 is referenced. This is likely a typo and should read Map 13. <br /> Please correct as necessary. <br /> 12. Page 2.05-10a (revised 10/30/89) should be re-submitted to be a one-sided page. <br /> 13. Figures 3 and 4 should have page numbers 2.05-12 and 2.05-13, respectively. <br /> 14. Page 2.05-16, Table 23 should be submitted as a one-sided page. <br /> 15. Pages 2.05-27 to 2.05-43 are either improperly numbered or missing numbers. Correct <br /> this information as necessary. <br /> 16. Page 2.05-43 should be a one-sided page. <br /> 17. Page 2.05-33 states Figure 6 depicts the final configuration of the RDA pile, with an <br /> access road running up the face. This contradicts later information and on-the-ground <br /> configuration. Please correct and remove information from this section as appropriate. <br /> 18. The original findings document issued for the New Elk mine states "The principle spillway <br /> is gated, but the gate will be opened a calculated amount to allow for automatic <br /> dewatering with a detention time of 24 hours." Does the permit indicate how much each <br /> gate will be open at all times? If so, please refer the Division to that information. If not, <br /> provide the appropriate permit information which addresses this issue. Unless all ponds <br /> can be demonstrated to be over-sized, the outlet gates should always be open to allow self- <br /> dewatering of the ponds. <br /> -8- <br />
