Section V <br /> Summary and Discussion <br /> This section consists of questions or comments raised as a result of the Division's review of the <br /> permit application and related materials. Please submit a technical revision responding to the <br /> issues identified below. <br /> Rule 2.03 Legal, Financial, Compliance and Related Information <br /> 1. Please provided updated information for Rule 2.03.4, with regard to surface ownership, <br /> resident agent, and corporate officer and director information. Include any changes to the <br /> appropriate maps as necessary. <br /> 2. Under Rule 2.03.9, the statement with regard to the insurance information being <br /> confidential should be removed, since insurance information does not meet the <br /> requirement of confidential information. <br /> 3. Rule 2.03.6(1) requires, among other information, a complete and detailed legal <br /> description to be included in the permit document. Please submit this information for <br /> incorporation into the permit. <br /> 4. Pages under section 2.03 of the permit are numbered incorrectly starting with 2.03-16. <br /> Please correct this information. Additionally, page 2.03-16 refers to Table 4, Other <br /> Licenses and Permits, but this information is included in Table 3a on page 2.03-18. <br /> Please correct as necessary. <br /> Rule 2.04 Information on Environmental Resources <br /> 5. Information at the bottom of page 2.04-24 which begins under "(2) Surface Water <br /> Information" should be removed as this is repeated on the next page. <br /> 6. The following pages are not identified with a page number: 2.04-6, 2.04-11. 2.04-41. <br /> Please correct and submit these pages. <br /> Rule 2.05 Operations and Reclamation Plans <br /> 7. Rule 2.05.3(2)(b) requires a detailed narrative and appropriate map(s) sufficient to <br /> describe all surface disturbances to be associated with the underground mining operation. <br /> The currently approved permit does not include descriptions of each of these surface <br /> disturbances. Discussion should be included for each disturbed area and include <br /> information on when this disturbance was created, specifically where it is located, what <br /> it is presently used for, and reference to the reclamation plan details as appropriate.
