Chapter I - Introduction and Legal Information <br />a) The operator should review this section and make any necessary <br />changes to provide current information. Contact people and other <br />such information should be updated. Compliance information could <br />also be updated to include only those violations received in the <br />past three years. This latter requirement may be waived until <br />permit renewal. <br />b) Expiration dates for leases and other required permits should be <br />included to verify that they are effective over the entire permit <br />term. Of particular concern is the special use permit, which <br />according to Division records expires in May, 1988. The operator <br />must specifically address the expiration of this permit. <br />c) Reference to pits 7 and 8 should be removed from Table 1: <br />2. These pages should be <br />deleted, with the current monitoring program as revised in TR-02 <br />(9/86) and Minor Revision No: 6 which revises the inactive <br />monitoring program inserted into the proper sections.
