III, Specific Comments and Requirements
1. Chapter I - Introduction and Legal Information
a) The operator should review this section and make any necessary
changes to provide current information. Contact people and other
such information should be updated. Compliance information could
also be updated to include only those violations received in the
past three years. This latter requirement may be waived until
permit renewal.
b) Expiration dates for leases and other required permits should be
included to verify that they are effective over the entire permit
term. Of particular concern is the special use permit, which
according to Division records expires in May, 1988. The operator
must specifically address the expiration of this permit.
c) Reference to pits 7 and 8 should be removed from Table 1:

The overburden and interburden-sample analysis from Pit 4, submitted
April 24, 1984, should be inserted in Chapter II: D. The narrative
on page II. D-6~should be amended to include these samples.
Pages II. F-16 a-c contain additional notes which update the surface
and ground water monitoring programs (2/85). These pages should be
deleted, with the current monitoring program as revised in TR-02
(9/86) and Minor Revision No: 6 which revises the inactive
monitoring program inserted into the proper sections. Th spoils
