Elam Construction, Inc. 10:49:42 a.m. 04-02-2005 3 /3 <br />April 1, 2005 <br />CDPH&E Report No. 2005-181 <br />Page 2 <br />• At approximately 2:50 P.M., Gayle Lyman notified the CDPHE Environmental <br />Emergency spill Reporting Line of the spilt. Information was provided to CDPHE <br />Personnel and Report No. 2005-181 was assigned to the incident. The owner of the <br />property was also notified of the incident along with the National Response Center. <br />The National Response Center provided their confirmation number of 754089. <br />LaFazge West, holder of a mining and reclamation permit No. M-1980-131, along <br />with the State Division of Minerals and Geology were also notified, since this <br />dischazge happened within a perntitted mine site. <br />• Clean-up procedures implemented for the spill include excavation of contaminated <br />material using Elam's on-site equipment and personnel. Components of the asphalt <br />plant will be removed from t~te affected area, as needed, to facilitate the clean-up of <br />the contaminated material. Initially, the excavated contaminated material has been <br />placed in a lined and bermed containment area. The contaminated material will <br />ultimately be treated on-site. Methods for on-site treatment will include evaporation <br />of volatile components and processing the material through Elam's on-site asphalt <br />plant. Once treatment of the soil is complete, samples of the material will be obtained <br />and tested at a qualified ]ab for Total Petroleum Hydrocarbons-Diesel, for <br />confirmation it is no longer contaminated <br />Some of the information presented above may vary slightly from that presented verbally to the <br />Department on Mazch 2$. This is due in pazt to additional information obtained from a more <br />extensive field investigation that was conducted after the verbal notification.
