'~
N`-l~~-r31
This report serves as written notification of a petroleum release from a storage tank in excess of
25 gallons. The dischazge occurred at the Bunn Ranch Gravel Pit near Craig Colorado. The
physical address of the gravel pit is 1848 East 151 Street, Craig CO. The property is owned by
Bob Quillen. Following is a sequence of events based on a site investigation:
At approximately 8:00 AM on Mazch 28, 2005, Mike Johanas, an employee of Elam
Construction, discovered diesel fuel spraying from a fuel line connecting a 6,000-
gallon mobile fuel storage tank .to the mixing drum of an asphalt hot plant. The
asphalt plant is owned and operated by Elam Construction. The valve was
immediately closed at the tank to prevent additional fuel from discharging. The
foreman was then notified of the fuel dischazge. Measures were immediately
implemented by site persommel to contain the dischazge. Although the spill was no
closer than 100-fee[ to any water source, an absorbent boom was placed in the water
as a precaution.
At approximately 8:30 A.M. Gayle Lyman, Property Manager with Elam
Construction, located in Grand Junction was notified of the incident. Mr. Lyman left
shortly thereafter, and arrived on-site at approximately 1:00 P.M. to investigate the
cause and extent of the spill. As stated eazlier, the dischazge of fuel was through a
flexible fuel line connecting a mobile fuel tank with an asphalt hot plant mixing
drum. The outer covering of the fuel line, in the area of failure, was not present. It
appeared, by the appeazance of the fuel line, that a small animal, or rodent of some
kind, gnawed at the outer covering of the fuel line until failure occurred. The ground
surface affected by the discharge was irregulaz in shape, but covering approximately
2500 square feet, to an es[emated depth of 18 to 24-inches. Based on information
obtained from site personnel, it is estimated that approximately 650 gallons of diesel
fuel was dischazged. The diesel did not come into contact with any water
