-8- <br />Report, TFS-5805, against the OSM LOC certified to the U.S. Depart-en*_. <br />Withdrawals of Federal funds are not to exceed the amount approved under <br />this Cooperative Agreement. Under provisions of Treasury Circular No. <br />1075, failure to adhere to these fund withdrawals and reporting <br />requirements may cause the unobligated portion of the LOC to be revoked. <br />ARTICLE V. REPORTS <br />Section 1. OSM requires the Recipient to use the <br />standardized "Outlay Report and Request for <br />Reimbursement for Construction Programs", SF- <br />271, to report construction costs of all <br />Construction Projects. <br />Section 2. OSM requires the Recipient of this Cooperative <br />Agreement to submit the Financial Status Report <br />and the Outlay Report and Request for <br />Reimbursement for Construction Programs <br />(original and two copies) not later than 30 days <br />after the end of each Federal quarter. The <br />final report is due 90 days after completion of <br />this Cooperative Agreement. <br />ARTICLE VI. MONITORING AND REPORTING OF PROGRAM PERFORMANCE <br />OSM requires the Recipient to submit a Performance Report, OSM-51, which <br />briefly presents the following of each program, function, activity, or <br />project involved: <br />Section 1. Comparison of actual accomplishments to the <br />goals established for the period, where the <br />output of this Cooperative Agreement can be <br />readily quantified. Such quantitative data <br />should be related to cost data for computation <br />of unit costs. <br />Section 2. Reasons why, if established goals were not met. <br />Section 3. Other pertinent information including analysis, <br />and other explanations of cost overruns or high <br />unit costs. <br />Section 4. The Performance Report (OSM-51) shall be <br />submitted with the frequency of reporting <br />indicated in Article 5, Section 2. <br />
