It <br /> remains outstanding at this time. <br /> Stipulation No. 13 <br /> APPROVAL OF MINOR REVISION NO. 83 FOR THE NEW ELK MINE IS <br /> CONDITIONED UPON THE RESOLUTION OF THE FOLLOWING: <br /> 1)KENTUCKY CITATION NO. C-98-081-414-001 (KY 4977004), ASSOCIATED WITH <br /> STAR FIRE MINING COMPANY; <br /> 2)KENTUCKY FORFEITURE, CESSATION ORDERS NOS. 050265 AND 050320 AND <br /> CIVIAL PENALTIES NOS. 052522, 052908 AND 053099 FOR PERMIT NO. 8980140, <br /> ASSOCIATED WITH CATAWABA INDUSTRIES; <br /> PROOF OF RESOLUTION SHOULD BE SUBMITTED TO THE COLORADO DIVISION <br /> OF MINERALS AND GEOLOGYAS SOON AS IT BECOMES AVAILABLE. <br /> Stipulation No. 13 was issued under conditional approval of Minor Revision No. 83. It <br /> remains outstanding at this time. <br /> Section IV <br /> Enforcement Actions <br /> This section is a review of the enforcement actions issued since the last permit renewal <br /> (RN-03) in March of 1999. The mine received one violation in December of 2000. No <br /> enforcement actions are currently outstanding. <br /> NOV#C-2000-011 <br /> This violation was issued for failure to maintain the Frontier Insurance Company <br /> Reclamation Bond #110345 in compliance with applicable Statutes and Regulations. <br /> The violation was terminated on January 12, 2001. Payment of the civil penalty of <br /> $1,000.00 was received by the Division on June 14, 2001.
