2. General Liability (Public Liability & Property Damage) (Minimum); <br />a. Combined single limit of S600,000 written on an occurrence basis; <br />b. Any aggregate will not be less than S1 million; <br />c. The contractor must purchase additional insurance if claims reduce the annual <br />aggregate below S600,000; <br />d. The State of Colorado must be named as additional insured on each <br />comprehensive general liability policy; <br />e. Certificates of insurance must be provided to the State within 10 working days <br />after receipt of Award; and, <br />f. Insurance shall include provision providing notification to the State 10 days <br />prior to cancellation of the coverage. <br />3. Automobile liability (Minimum). The contractor must carry a minimum of 5600,000 <br />combined single limit. <br />Work Description <br />Hourly bids are being solicited for work to be performed at the Fruita site in the Bookcliffs <br />north of Fruita, Colorado. All equipment rental hours will be conducted per the attached 19.0 <br />Equipment Rental specifications. The Project Manager must be on site for all equipment rental <br />hours. The Project Manager will be available during regular business hours 18:00 A.M. to 5:00 <br />P.M.) on weekdays. Equipment Rental time will not be scheduled on weekend days or <br />holidays without prior approval of the Project Manager. <br />1) D-4C Dozer w/PAT & R Hours -Bids are being solicited fora 78 fhp dozer with ripper <br />D-4C or equivalent with power angle tilt and ripper. Mobilization will be included in the <br />submitted hourly rate. Work will consist of the general roughening and construction <br />of contour furrows of an overcast waste slope presently exhibiting slopes at <br />approximately 1.5:1 to 2:1 (H:V). It is assumed that another piece of machinery <br />equipped with a winch is going to be required to assist the dozer in repeated climbs <br />up the slope for surface preparation for revegetation. <br />2) Reveoetation -After contour furrows have been constructed and the surface <br />manipulated according to the direction of the Project Manger, all disturbed areas will <br />be reseeded with seed provided by the State. Seed will be sown uniformly on the <br />prepared areas. Broadcast seed will be covered about 1/4 to 1/2 inch deep by a <br />method acceptable to the Project Manager. All reseeded areas will be mulched with <br />hay or straw mulch, pest-free or weed-free to the extent possible. Mulch will be <br />supplied by the Contractor. Approximately 30 percent by weight of the mulch material <br />will be 10 inches in length or longer. Hay or straw mulch will be applied uniformly at <br />the rate of two tons per acre. If hydromulching is chosen, hydromulch will be applied <br />at the rate of 1 1 /2 tons per acre. Mulch will be crimped (stabilized or anchored) into <br />the soil by hand with a suitable tool on 18" centers. The cost for revegetation will be <br />based on lump sum. <br />2 <br />
