<br />-3- <br />Stipulation No. 4: <br /> <br />WITHIN 60 DAYS OF PERMIT ISSUANCE, THE PERMITTEE MUST SUBMIT A PLAN FOR <br />SEDIMENT CONTROL FOR THE WEST AREA. IF THIS PLAN DOES NOT INCLUDE THE <br />USE OF SEDIMENT PONDS, A REQUEST MUST BE MADE FOR ALTERNATIVE SEDIMENT <br />CONTROL MEASURES. THE REQUEST MUST JUSTIFY WHY SEDIMENT PONDS ARE NOT <br />NEEDED TO MEET EFFLUENT LIMITATION FOR THESE AREAS IN ACCORDANCE WITH <br />RULE 4.05.2(3). IN ADDITION, THE PERMITTEE MUST DESCRIBE WHAT <br />ACTIVITIES OVER THE NEXT FIVE YEARS WILL OCCUR IN THE WEST AREA, AND THE <br />LOCATION AND DESIGNS OF DITCHES AND CULVERTS USED TO ROUTE RUNOFF WATER <br />TO THE RIVER. THIS PLAN WILL BE SUBMITTED AS A REVISION TO THE PERMIT. <br />Stipulation No. 5: <br />WITHIN 60 DAYS OF PERMIT ISSUANCE, THE PERMITTEE SHALL INSTALL STAFF <br />GAGES IN SEDIMENT PONDS 006 and 007 THAT INDICATE THE LEVEL OF SEDIMENT <br />STORAGE VOLUME AS A PERCENTAGE OF THE TOTAL SEDIMENT STORAGE VOLUME. <br />Stipulation No. 6: <br />WITHIN 60 DAYS OF PERMIT ISSUANCE, THE PERMITTEE MUST REVISE THE PERMIT <br />APPLICATION TO INCLUDE ALL PERTINENT INFORMATION ON DISCHARGE FROM THE <br />SEDIMENT CONTROL PONDS. <br />Stipulation No. 7: <br />THE PERMITTEE SHALL CONDUCT A PRELIMINARY SPRING AND SEEP SURVEY USING <br />AERIAL PHOTOGRAPHY OF THE PERMIT AND ADJACENT AREAS. THE PRELIMINARY <br />SPRING SURVEY SHALL CONSIST OF A MAP OF THE PERMIT AND ADJACENT AREAS <br />WHICH WILL SHOW THE LOCATION OF ALL POTENTIAL SPRINGS AND SEEPS <br />IDENTIFIED ON THE AERIAL PH070GRAPHS. THE PRELIMINARY SPRING SURVEY <br />SHALL BE SUBMITTED TO THE DIVISION WITHIN 30 DAYS OF THE PERMIT ISSUANCE <br />AND BEFORE THE COMMENCEMENT OF MINING IN T.HE ALL€N MINE. <br />Stipulation No. 8: <br />THE PERMITTEE SHALL VERIFY THE LOCATIONS OF ALL SPRINGS AND SEEPS <br />IDENTIFIED IN THE PRELIMINARY SPRING SURVEY THROUGH A FIELD SURVEY. <br />THIS FIELD SPRING SURVEY SHALL ALSO INCLUDE A DESCRIPTION OF EACH SPRING <br />OR SEEP KEYED TO A LOCATION MAP. THE DESCRIPTION SHALL IDENTIFY THE <br />POTENTIAL OR OBSERVED WATER SOURCE OF EACH SPRING OR SEEP (i.e., <br />SANDSTONE OUTCROP, ALLUVIUM, FAULT ZONE, FRACTURE ZONE, ETC.); THE <br />MEASURED OR ESTIMATED FLOW RATE OF EACH SPRING OR SEEP; AND THE FIELD <br />MEASUREMENT OF pH, ELECTRO-CONDUCTIVITY, AND TEMPERATURE FOR EACH SPRING <br />OR SEEP. THIS SPRING SURVEY SHALL BE CONDUCTED IN MARCH OR APRIL OF <br />1984, WHEN WEATHER PERMITS ACCESS TO THESE AREAS. RESULTS OF THE FIELD <br />SPRING SURVEY SHALL BE SUBMITTED TU THE DIVISION BY JUNE 1, 1984. <br />
