Summary <br />The Review Process <br />The Southfield Mine is a reclaimed underground coal mine that is owned and operated by Energy <br />Fuels Coal, Incorporated (EFCI). A permit to operate the mine was originally issued to <br />Dorchester Coal Company under the name of Dorchester No.l Mine on July 30, 1984. EFCI <br />bought the operation in Mazch 1985 and changed the name of the mine to Southfield. The <br />transfer of the permit was approved by the Division on Mazch 22, 1985. <br />The permit was renewed and reissued on January 5, 1990. In April of that same yeaz an <br />application for permit revision was received to include an additional 1,235 acres to the permit <br />azea. The permit revision was reviewed and eventually approved by the Division on February 1, <br />1991. <br />Pursuant to Rule 2.08.5, the Division, on October 6, 1993, notified Energy Fuels Coal, Inc. of the <br />need to renew the permit in a timely manner. The Division received the renewal application on <br />February 10, 1994. The application was deemed complete on February 14, 1994. The adequacy <br />review was in two phases, with completed review by June 20, 1994. Response to the first phase <br />of the adequacy review was on Mazch 31, 1994. Response to the second phase was begun in <br />April 1994 with completion on June 16, 1994. A third component of the permit renewal, bond <br />evaluation was begun in April 1994 and completed on June 17, 1994. Final approval of the <br />renewal occurred on June 20, 1994. <br />Pursuant to Rule 2.08.5, the Division, on 8 October, 1998, notified Energy Fuels Coal, Inc. of the <br />need to renew the permit in a timely manner. The Division received the renewal application on <br />31 January 1999. The renewal application was deemed complete on 2 February 1999 and a <br />preliminary adequacy letter was sent out on 21 June 1999. The responses to the adequacy were <br />received on 14 May 2001. Final approval of the renewal occurred on 22 3une 2001. <br />Pursuant to Rule 2.08.5, the Division, on 31 October 2003, notified Energy Fuels Coal, Inc. of <br />the need to renew the permit in a timely manner. The Division received the renewal application <br />on 16 January 2004. The renewal application was deemed complete on 26 January 2004 and a <br />preliminary adequacy letter was sent out on 27 Apri12004. The responses to the adequacy were <br />received on 20 May 2004. A second adequacy letter was sent to EFCI on 21 June 2004. This <br />letter regazded resolving stipulations 4, 6, 14, 15 and 18. Responses to this letter were received <br />on 22 July 2004. Final approval of the renewal occurred on 10 September 2004. <br />The Division currently holds a surety bond from National Union Fire Insurance Company in the <br />amount of $380,431.00. The current estimated cost for the Division to complete reclamation at <br />the Southfield Mine, in the event of bond forfeiture, is $223,856.00. The current bond amount of <br />$380,431.00 was set statutorily through the Phase I Bond Release SL-02 and cannot be decreased <br />without the submittal and approval of another bond release. <br />Southfield Mine Permit Renewal 04 13 September 10, 2004 <br />
