<br />When there is a pipeline release, the following information will be documented: <br />i <br />• Type of product released (i.e., which line had the release) <br />• Location of release <br />• Date, time, and duration of the release <br />• Estimate of quantity released <br />• Cause of release, if known <br />• Number of deaths or injuries, if any <br />' Safety precautions taken, if any. <br />Incident Investigation <br />As soon as possible after a pipeline incident, the Plant Manager will commence an <br />incident investigation. The Plant Manager may designate a qualified employee to <br />conduct the investigation. The designee must be knowledgeable in incident <br />investigation procedures and have conducted previous investigations. <br />Depending upon the extent and seriousness of the incident, consideration should be <br />' given to assembling an incident investigation team. The team may include plant <br />management, operation supervisors, maintenance supervisors, operators, technicians, <br />engineering personnel, regulatory compliance personnel, and key individuals from the <br />' area in which the incident occurred. <br />The task of the investigation team is to examine data regarding the incident. <br />' Operational information such as pressure, flow rate, valve openings and closures, and <br />pump operation for the time leading up to the incident will be included in the <br />investigation, as appropriate. An on-site investigation will normally be required, with <br />importance placed on examining failed pipe or components. <br />If appropriate, recommendations will be made by the designated employee or team to <br />' the Plant Manager for procedural modifications, changes to facilities, or operations in <br />order to prevent recurrence of the problem. These recommendations will be evaluated <br />for operational and engineering soundness, and an action plan and schedule for <br />' implementation will be set forth. <br />Records of abnormal operating episodes and the response of operations personnel to <br />' the abnormal occurrences will be reviewed periodically by management personnel to <br />determine the effectiveness of procedures and responses.
