Phase Nc Summary No. 8 Page 2 043-2170 <br />Ames did not produce any bentonite-amended SLF at the screen and pug mill plant due to inclement <br />weather. Ames continued to produce Drain Cover Fill (DCF) material at the screen plant located neaz <br />the secondary crusher. <br />Poly-Flex did not deploy or seam any new geomembrane panels during this reporting period. Poly-Flex <br />continued repair and testing of previously deployed and seamed panels from the prior week. Poly-Flex <br />did not perform any destructive fusion or extrusion peel and sheaz testing during this reporting period. <br />Ames continued placement and grading of DCF material into the Phase IVc OSA in the western portion <br />of the VLF, Saddle Fill area. Ames placed DCF material onto approved geomembrane panels and High <br />Volume Solution Collection (HVSC) piping per the project plans. <br />A map of the azea worked during this reporting period is provided in Attachment C. <br />Field Monitoring and Testing Activities <br />Golder observed constmction activities associated with Phase IVc earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed excavation and construction of the LDT in Phase IVc for trench dimensions, slope, <br />and materials used. <br />Golder observed seam testing (trial seams, pressure, vacuum and destructive) of previously deployed <br />geomembrane. Geomembrane tested in the Phase Nc OSA met the project specifications. <br />Golder visually observed the placement of DCF in the Phase IVc OSA azea. Golder CQA monitors <br />(one monitor per dozer when dozers are greater than 50 feet apart) observed DCF placement and <br />verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic (i.e., Caterpillar <br />Mode1740 articulated trucks). <br />Golder observed placement of HVSC piping in the Phase IVc OSA, verifying installation and layout <br />per the project plans. <br />Golder visually observed removal of DCF on July 2 from the Phase IVc OSA azea to allow for <br />inspection of the Phase IVc geomembrane due to equipment damage. Golder mazked the defect <br />damage (31C) caused to the geomembrane, observed the repair and non-destructive testing, and <br />passed the defect during this reporting period. <br />Laboratory Tests <br />Golder obtained no QC samples during this reporting period. <br />Golder obtained one QA sample of DCF (DCFc-03) for laboratory index testing.
