2004 Midterm Review <br />Rule 4.05.4 Stream Diversions <br />In compliance by exception. There aze no stream diversions at the site. <br />Rule 4.05.5 Sediment Control Measures <br />In compliance. Although sediment control structures remain in place, the entire site has been <br />granted Phase II Bond Release. <br />Rule 4.05.6 Sedimentation Ponds <br />In compliance. Although not required, one sediment pond, Sediment Pond A, is still onsite. It has <br />a combined spillway. It is a combination incised and low berm facility. The pond is inspected on <br />a quarterly and yeazly basis. <br />Rule 4.05.7 Dischazge Structures <br />In compliance by exception. The spillway is concrete lined with a concrete outlet. No additional <br />features aze required. <br />Rule 4.05.8 Acid-forming and Toxic-forming Spoil <br />Spoil was not generated at this underground mine. <br />Rule 4.05.9 Impoundments <br />In compliance. Sedimentation Pond A is in compliance with the requirements of this rule, <br />Rule 4.05.10 Underground Mine Entry and Access Discharges <br />In compliance by exception. Although there is a drain provided through the Portal A seal, there <br />has been no recorded dischazge at the site. <br />Rule 4.05.11 Ground Water Protection <br />In compliance. Drill holes and wells were sealed. Although the applicant prepared for more <br />extensive protection of ground water, premature closure of the mine vacated implementation. <br />Rule 4.05.12 Protection of Ground Water Recharge Capacity <br />In compliance.
