<br /> <br />Mid Term Review <br /> <br />Hayden Gulch Mine - Permit No. C-80-003 <br />Tnis document presents the results of the Division's mid-term review for the <br />Hayden-Gulcn Mine. This review was conducted to fulfill the requirements of <br />Rules 2.08.3, 2.06.2(9), 2.06.3(4), 2.06.5(3), 2.06.7(5) and 3.02.2(4). <br />Rule 2.08.3 requires that the Division conduct a review of each permit issued <br />prior to its mid-term. Based on this review, for good cause shown, the <br />Division may require reasonable revision or modification of the permit <br />provisions to ensure compliance with the Act and Regulations. <br />Rules 2.06.2, 2.06.3, 2.06.5 and 2.06.7 require that experimental practices, <br />mountaintop removal variances, variances from AOC, and variances from <br />contemporaneous reclamation oe reviewed by the Division where applicaole. <br />Tne Hayden Gulch permit noes not have any operations under these categories. <br />Rule 3.02.2(4) requires that the Division review the amount of the bond and <br />the terms of acceptance of the bond every 2 1/2 years. This is done in <br />Section III of tnis document. <br />Tne mid-term review consisted of a detailed review of the Havden Gulch <br />findings document, Dermit revision findings documents, permit renewal findings <br />document, and the permit application to identify any potential problems. The <br />Division also reviewed all subsequent revisions and stipulation responses to <br />ensure that all permit commitments and conditions are being followed. The <br />annual reclamation reports were also reviewed for completeness and accuracy. <br />The hydrologic monitoring data was reviewed to assess the adequacy of the <br />monitoring plan and determine any hydrologic impacts which may have occurred <br />as a result of the mining disturbance. <br />Tnis document is organized as follows: Section I includes a summary of <br />cnanges and updates that need to oe incorporated into the permit application <br />or submitted to the Division. Section II summarizes the revisions to the <br />permit and also lists the stipulations and responses since renewal. Section <br />III discusses the current state of bonding at the H-G Mines as required under <br />Rule 3.02.2(4). <br />There are several areas in the permit application whicn require revisions <br />to the text or exhibits to reflect the current status of the mine. The <br />section identifying otner licenses and Dermits 2.03.10 appears to be very <br />outdated and may no longer reflect the current state of the mine. Please <br />review this section and update accordingly (i.e. New NPDES Permit, do you <br />need a blasting Dermit?). Is the certificate of insurance still <br />current? If so, please provide proof. Information pertaining to the <br />revised shrub density standard (TR-53) has not been submitted or <br />incorporated into the permit application based on Division files. The <br />revised pages should be submitted with H-G's response. The water level <br />for well UC-2 was not included in the 1988 Reclamation Report. H-G <br />should therefore provide the missing data. <br />
