WiII the extracted material be hauled offsite or used on the same parcel of <br />property where the material is extracted? Both <br />5. What on site use will be made of the extracted material? The gravel not used in <br />the Highway 12 road project will be used on the on site roads. The topsoil will be <br />used to reclaim the area disturbed. <br />6. If the material is hauled offsite, where will it be hauled and what is the intended <br />use? Upon being crushed the material will be used in the asphalt on Highway <br />12. <br />7. What is the approximate area extent of the proposed extraction in acres? 1.5 <br />Acres <br />8. To what approximate depth will the extraction extend? Approximately 12-18ft. <br />9. In cubic yards, approximately how much material will be removed? 32,000 CY of <br />gravel <br />10. By what method(s) will the material be extracted? The material will be removed <br />with front end loaders, dump trucks, dozers, and excavators. <br />11. Will either the landowner or the mine site operator receive any type of <br />compensation, i.e., monetary, in-kind, etc., from the proposed material <br />extraction? The mine site operator will receive compensation for the removal and <br />haulage of the material to the crushing site. <br />12. Please supply a copy of any documents that will ensure that the area of <br />extraction will be reclaimed to some beneficial land use once extraction activities <br />have been completed. Area is part of coal Permit No. C-96-084 and will be <br />reclaimed per Permit No. C-96-084. <br />13. Do you have permits for this activity from any other governmental agencies, and, <br />if so, what are they? No <br />14. Are there state/federal/local agencies participants in terms of funding (yes}-(no), <br />design (yes)-(no).-What are the percentages?
