RE: Renewal of Summit County Conditional Use Perrnlt and Amendments <br />for Maryland Creek Ranch Sand and Gravel <br />Power and other utility lines have been installed underground <br />A dry hydrant has been installed and certified by the Lake Dillon Fire District. It is located near <br />the Maryland Creek Ranch House. <br />A clean sweep area is in use and located near the exit, at the outbound scale, so that drivers <br />can clean off their trucks prior to leaving to site. <br />Reoortina Requirements <br />We have annually submitted a report containing the information required in our conditional use <br />permit including annual gross tonnage, the DMG annual Report, documentation on the concrete <br />and asphalt plants, Section 404 monitoring report, CDPHE Water Quality Control Division <br />annual report, and CDOT traffic report. <br />We already are required by state and federal agencies to file annual reports. Therefore, we <br />would like to request removal of the county annual reporting requirements. <br />Graphic Material <br />Mining and reclamation maps/plans included in this submittal are listed below. There are six <br />24"x36" copies and six 11"x17" copies of all graphic materials, as Planning Staff requested. <br />- Exhibit C -Mining Plan (1 sheet) <br />- Exhibit F -Reclamation Plan (1 sheet) <br />- Lake 3 Reclamation Plan (2 sheets) <br />The mining plan shows details of processing area with the approximate locations of the shop <br />and office buildings, parking areas, and various processing equipment and stockpiles. <br />Topography and natural features are also shown, as well as stormwater detention ponds, berm <br />placement and cross-sections, and access roads. The reclamation plans give landscaping <br />details including lakeshore undulations, seed mix, and tree and shrubbery cluster placement. <br />Information required for aublic hearin <br />At time of submittal, Ilst of property owners and malfing address located 300 feet from <br />boundary of project site. <br />The list of property owners and their mailing addresses follows. An affidavit confirming that the <br />list was gathered at the Summit County Assessor's Office on July 3, 2001, is also attached. <br />At the time of the hearing, affidavit that signs advertising the public hearing were posted <br />on the protect site at least 10 days prior to the hearing. <br />L. G. Everist, Inc., will post signs advertising the public hearing at least 10 days prior to the <br />hearing.
