Long <br />Documented Telephone Quotes, Bid Number: PKA-7-1016 Dirisiun Dneclor <br />Enclosed is a brief description, maps and bid schedule for the Peacock Mine Maintenance <br />Project. The reclamation bond held by the State has been revoked and the State is conducting <br />maintenance reclamation at the site. <br />Site Showing <br />There will NOT be a pre-bid meeting. <br />Bid Submission and Evaluation <br />Bids shall be submitted by 4:00 p.m. on May 14, 1996 by mail or fax to Maggie VanCleef. <br />The attached bid schedule must be submitted by all bidders. Award will be made for lowest <br />grand total bid for the site. <br />Bid Award <br />The apparent low bidder must submit a certificate of insurance with the following limits: <br />Standard Worker's Compensation and Employer Liability, as required by State Statute <br />including occupational disease, covering all employees at the work site. <br />2. General Liability (Public Liability & Property Damage) (Minimuml; <br />a. Combined single limit of 5600,000 written on an occurrence basis; <br />b. Any aggregate will not be less than S1 million; <br />c. The contractor must purchase additional insurance if claims reduce the annual <br />aggregate below 5600,000; <br />d. The State of Colorado must be named as additional insured on each <br />comprehensive general liability policy; <br />e. Certificates of insurance must be provided to the State within 10 working days <br />after receipt of Award; and, <br />f. Insurance shall include provision providing notification to the State 10 days <br />prior to cancellation of the coverage. <br />
