The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />PB1. The operator is constructing a cinder block and cement 10-15-97 <br />wall outby from the wooden bulkhead to stop the leak. PB or PV PB <br />TOPIC(S) PW <br />Corrective Action: Hefore continuing with this <br />activity, submit and get approved, a plan to seal the <br />leak as a revision to the permit. Include in the plan, <br />provisions to salvage the existing discharge assembly <br />or install a new one. <br />PB2. Spray evaporatove system may not be functioning as 10-15-97 <br />approved. PB or PV PB <br />TOPIC(S) PW <br />Corrective Action: Submit verification that this system <br />meets the capacity as designed by Richard Fox and <br />approved by the Division. <br />
