(ii) all monitoring information, including all calibration and maintenance <br />records and all original chart recordings or digital files for <br />continuous monitoring instrumentation and copies of all reports <br />required by this permit for a period of at least five (5) years from the <br />date of the sample, measurement or report throughout the operating <br />life of the well. <br />(b) The permittee shall continue to retain such records after the retention period <br />specified in paragraphs (a) (i) and (ii) above unless he delivers the records to <br />the Director or obtains written approval to discazd them. <br />(c) The permittee shall maintain copies (or originals) of all pertinent records <br />jPart II, Section D. 1. The permittee shall submit Quarterly Reports to the Director <br />summarizing the results of the monitoring required by Part II, Section D. 1. (a), (b), <br />and (c) of this permit. The report shall include the monthly average, maximum, and <br />minimum measured values for injection pressure, flow rate and volume, and <br />annulus pressure. The operator shall also provide summary graphs covering the <br />reporting period of the injection pressure, the annulus pressure, and the injection <br />rate. Copies of the analytical results for the samples of injected fluids, and records <br />of any major changes in chazacteristics or sources of injected fluid shall be included <br />in the Quarterly Report. <br />The Quarterly Reports shall include the results and associated documentation of any <br />mechanical integrity testing, pressure falloff testing, well workover, or well logging <br />completed during the period covered by the report. <br />The first Quarterly Report shall cover the period from the effective date of the <br />permit through the end of that quarter. Subsequent Quarterly Reports for a yeaz <br />shall cover the periods of: January 1 through Mazch 31; April 1 through June 30; <br />July 1 through September 30; and, October 1 through December 31. Each Quarterly <br />Report shall be submitted to the Denver Office by the 15th of the following month. <br />Appendix B contains Form 7520-8 which maybe copied and used to submit the <br />quarterly summary of monitoring. <br />EPA Draft Permit No. CO 10932-04664 Page 16 of 77 <br />
