representative nature of the reported data. <br />(c) Continuous monitoring of injection and annulus pressure. The <br />instrumentation shall be capable of recording at least one value for each of <br />the pazameters at least every fifteen (15) seconds. Initially, recordings <br />should be made once every ten (10) minutes. Monitoring must occur <br />whether or not fluids aze being injected. This information shall be analyzed <br />in the first annual report to determine if this frequency is representative of <br />the injection activity. A minor modification to the Permit shall be made to <br />increase the frequency of recording if the variability of the injection pressure <br />and annulus (as warranted by the data results) affects the representative <br />nature of the data. A, minor modification to the Permit maybe made to <br />decrease the frequency of recording if the Director deternunes that the <br />fluctuation of the parameters is such that less frequent data collection would <br />not significantly affect the representative nature of the reported data. <br />2. Monitoring Information. Records of any monitoring activity required under this <br />permit shall include: <br />(a) The dates, exact place, and the time interval of sampling, monitoring, or <br />field measurements; <br />(b) The name of the individual(s) who performed the sampling or <br />measurements; <br />(c) The exact sampling method(s) used to take samples; <br />(d) The date(s) laboratori analyses were performed; <br />(e) The name of the indi~ridual(s) who performed the analyses; <br />(f) The analytical techniques or methods used by laboratory personnel; and <br />(g) The results of such analyses. <br />Recordkeepine. <br />(a) The perrnittee shall retain records concerning: <br />(i) the nature and composition of all injected fluids until three (3) yeazs <br />after the completion of plugging and abandonment which has been <br />carried out in accordance with the Plugging and Abandonment Plan <br />shown in Appendix C. <br />EPA Draft Permit No. CO 10932-04664 <br />Page 15 of 77 <br />
