(c) The wellbore should be filled from the CIBP to 320 feet BGS with 10-pound-per- <br />gallon calcium chloride brine containing a gelling agent to increase viscosity; <br />(d) An intermediate plug will be set by perforating the casing at 1,100 feet BGS and <br />squeezing sufficient Class B Type II or Class G cement to establish a 100 plug in <br />the annular space adjacent to the 41/2 inch casing and inside of the casing. <br />(e) Setting the top plug requires perforation of the 4-1/2 inch casing at 315 feet and <br />squeezing either Class B Type II or Class G cement to the surface between the 4-1/2 <br />inch casing and the 8-5/8 inch surface casing. The 4-1/2 inch casing must be filled <br />with identical cement to the surface. <br />(f) After the wellbore is plugged the Permit requires cutting off the 8-5/8 inch and 4- <br />1/2 inch casings 1 to 3 feet below ground surface. A steel cap dry hole marker is <br />required to be welded on the 8-5/8 inch casing. The surface must then be restored <br />to la~idowner andJor County requirements. <br />Inactive Wells. <br />(Condition 3) <br />The Permit requires that after a two (2) year period of injection inactivity, the permittee <br />must plug and abandon the well in accordance with the Plugging and Abandonment Plan, unless <br />the permittee: <br />(a) has provided notice to the Director; and <br />(b) has demonstrated that the well will be used in the future; and <br />(c) has described actions or procedures, satisfactory to the Director, that will be taken <br />to ensure that the well will not endanger underground sources of drinking water <br />during the period of temporary abandonment. <br />Plugging and Abandonment Reoort. <br />(Condition 4) <br />Within sixty (60) days after plugging the well, the permittee shall submit a report on <br />Form 7520-13 to the Director. The report shall be certified as accurate by the person who <br />performed the plugging operation and the re))ort shall consist of either: (1) a statement that the <br />well was plugged in accordance with the plan; or (2) where actual plugging differed from the plan, <br />a statement that specifies the different procedures followed. <br />Statement of Basis for EPA Permit No. CO 10932-04664 <br />Page 17 of 18 <br />
