The Permit requires that the average; maximum and minimum monthly values of injection <br />pressure, flow rate and volume, and annulaz pressure be reported quarterly, along with the data <br />from the fluid analyses. The operator shall also provide summary graphs covering the reporting <br />period of the injection pressure, the annulus pressure, and the injection rate. Copies of the <br />analytical results for the samples of injected fluids, and records of any major changes in <br />chazacteristics or sources of injected fluid shall be included in the Quarterly Report. <br />The Quarterly Reports shall include the results and associated documentation of any <br />mechanical integrity testing, pressure falloff testing, well workover, or well logging completed <br />during the period covered by the report. <br />The first Quarterly Report covers the period from the effective date of the permit through <br />the end of that quarter. Subsequent Quarterly Reports for a year cover the periods of: January 1 <br />through Mazch 31; April I through June 30; July 1 through September 30; and, October 1 through <br />December 31. Each Quarterly Report must be submitted to the Denver Office by the 15th of the <br />following month. The Permit requires submittal of the information on Form 7520-8 with <br />attachments as needed. <br />PART II, Section E PLUGGING AND ABANDONMENT <br />Notice of Plugging_and Abandonment <br />(Condition 1) <br />To provide sufficient time for the Director to witness the well plugging, if deemed <br />necessary, the Permit requires that the Director be notified forty-five (45) days before <br />abandonment of the well. <br />Pluggin¢ and Abandonment Plan. <br />(Condition 2) <br />The plugging and abandonment plan (Appendix C of the Permit) submitted by the <br />permittee, consisted of two (2) plugs (this counts the placement of cement in the surface casing <br />and inside of the 4-1/2 inch casing at the top of the well as one plug). This plan has been reviewed <br />and approved by the EPA with one modification. To ensure protection of USDWs in the Wasatch <br />from upwazd movement of fluids adjacent to the uncemented longstring casing, an intermediate <br />plug will be required, the resulting composite plugging Plan required by the Permit is as follows: <br />(a) Immediately prior to plugging and abandoning the Getty # 1-7L disposal well, the <br />retrievable tension-type packer, the 3,891 feet of 2-3/8 inch tubing will be released <br />and removed from the wellbore; <br />(b) The operator is required to enter the wellbore with a tubing string to the bottom of <br />the 4-1/2 inch casing and condition the wellbore. A cast~ron bridge plug (CIBP) <br />should be set in the 4-1/2 inch casing at 3,875 feet BGS with a 100 foot cement <br />plug on top of the CIBP using Class B type II neat cement or an equivalent Class G <br />cement; <br />Statement of Basis for EPA Permit No. CO 10932-04664 Page 16 of 18 <br />
