The report was originally sent to <br />a Mr. Steve Gunderson, Director of Emergency Management Unit at the <br />following address: <br />Mr. Steve Gunderson <br />Director fo Emergency Management Unit <br />Colorado Department of Health <br />4210 East 11th Avenue <br />Denver, CO 80220 <br />The cover letter was dated June 9, 1992. <br />I have also included the TCLP test from the ACZ laboratory that was <br />performed on the spilled material.
