ALLUVIAL WELL ABANDONMENT <br />The prior operators of the Blue Ribbon Mine utilized an al- <br />luvial well for industrial purposes. The location of the well is <br />shown in the permit on Exhibit A. The alluvial well was not <br />abandoned appropriately during the 1986 reclamation of the Blue <br />Ribbon Mine site. <br />On August 14, 1991, I physically located the alluvial well. <br />It was an open, 6-inch diameter hole that had been filled with <br />cuttings/earth sealant to within 15 feet of the surface. On that <br />same date I placed a 12-foot thick cement plug in the well then <br />backfilled the well the remaining 3-feet with earth. <br />I hereby certify that the alluvial well was abandoned in ac- <br />cordance with the water well cross section presented on page 116 <br />of the permit, Plate 17, revised August 15, 1991. <br />James E. ~ ..,~SIF,Date <br />P .
