As discussed, this is our request to confirm our understanding that Rule 1.2 <br />exempts the proposed activities on behalf of Forest Lakes Municipal District (FLMD). <br />Here are the answers to your questionnaire, as much as possible from other documents <br />related to the FLMD Waste Water Treatment Plant (WWTP) Expansion and Upgrade <br />Project (the "Project"): <br />1. Legal Description of Project: <br />Location: Section 3, T35N, R7W, 10"' (NM) P.M., La Plata County, Colorado. <br />Directions to site: From Bayfield (intersection of US 160 and CR 501) go seven miles <br />north on CR 501 to FLMD Road, turn left past Fire Station and enter gates to <br />construction site. <br />2. Site ownership and special requirements related thereto: Site is owned and <br />operated by the Forest Lakes Metropolitan District, PO Box 440 Bayfield, CO 81122, <br />970-884-2925. FLMD is a governmental entity. <br />3. Type of materials to be extracted and physical nature of the site: <br />Fill materials for use in construction project; topsoil for reclamation of extraction site and <br />other areas. The site is a terrace on the east side of the Pine River Valley, <br />approximately 30-40 feet above and 1/4 mile from the Pine River. <br />4. Extracted material will be used on the same parcel or contiguous parcel of <br />property. <br />5. The major use of the material will be to construct berms to divide the existing <br />WWTP polishing pond into three sections, for necessary fill in and around the <br />construction site, for reclamation (landscaping) of adjacent areas, and similar uses. <br />6. No material is planned to be hauled off-site. <br />7. The actual limits of extraction are proposed to be between 1.0 and 1.5 acres. <br />
