,~. r ~? <br />Mr. T.M. Russell - 2 - January 10, 1991 <br />5. If any soil stockpiles exist on site, please describe how these <br />will be stabilized from wind and water erosion. We would recommend <br />seeding them for both nutrient protection and reduction of erosive <br />losses. <br />6. Please describe the height, length and backwall angles of any <br />highwalls. If these highwalls have been benched, please describe <br />the height, length and width of these benches as well. <br />7. Please describe the width, length and gradients of all permitted <br />access roads. Please describe if these roads require water bars, <br />culverts or cut-outs along roadside berms for surface water <br />control. If any of these controls are required, please describe <br />how often they will be inspected. <br />8. Please describe all equipment that will be on site during active <br />operations, and what equipment will remain on site during inactive <br />periods. <br />9. Please describe how fuels, lubricants, explosives, and any other <br />required chemicals will be stored during both active and inactive <br />periods. <br />10. Please discuss whether the site will be fenced and gated during <br />inactive periods to prevent curious hikers, tourists, neighbors, <br />etc. from trespassing. Please describe any potential safety <br />hazards that exist at the site that could develop into problems <br />during inactive periods of operation, such as old mine shafts or <br />tunnels, natural caves, crumbling or ravelling highwalls or <br />benches, unstable historic wood structures, deep trenches, and <br />water-filled processing/settling ponds. <br />11. Please describe the number of personnel that will be on site during <br />both active and inactive periods. <br />12.
