<br />4.2 Inspection, Maintenance and Recordkeeping <br />Daily vehicle and equipment inspections are standard operating procedures. Signs of leaks or other <br />potential problems are reported to the mechanics for repair. Preventative maintenance is conducted <br />on all equipment with records kept by the company mechanic. <br />A comprehensive inspection of the site will be conducted no less than twice a year, in the spring and <br />fall. A sample inspection form is attached to the SWMP and may be modified to reflect changes in <br />the plan. Inspection will include all structural and non-structural BMP's, required maintenance, and <br />maintenance completion dates. If unforeseen erosion occurs, the SWMP will be revised as <br />necessary to maintain good stormwater quality. Any spill that occurs must be documented on a spill <br />report (see attached spill report form). <br />A copy of this Stormwater Management Plan and the inspection reports will be available at the site <br />for inspections by Colorado Department of Public Health and Environment (CDPHE) or Routt <br />County Department of Environmental Health personnel. In addition, an annual SWMP report is <br />required by the CDPHE on a form that they provide. This report will require a summary of the two <br />comprehensive inspections and details of any revisions to the plan as outlined in the permit <br />requirements. Copies of the annual report shall be maintained for at least three years. <br />Based on the results of the comprehensive inspections, any pollution prevention deficiencies will <br />be addressed through revisions of the SWMP within two weeks of the inspection. The SWMP <br />revisions shall provide for implementation of control measures or Best Management Practices. <br />Implementation will occur in a timely matter and will not be greater than 90 days after the inspection. <br />4.3 Employee Training <br />The SWMP administrator or designated training personnel shall review environmental and safety <br />procedures as required by the Mining Safety and Health Administration. This training will be <br />conducted to inform personnel of procedures to be followed so the operation is in compliance with <br />all permits. This training may be combined with other training. Some of the topics that may be <br />included are: <br />• Spill prevention and response <br />• Good housekeeping techniques <br />• Material management practices <br />• Sediment and erosion prevention <br />
