II. SUMMARY OF REVISIONS AND STIPULATIONS <br />Stipulations <br />Two stipulations were attached to the June 14, 1993 renewal permit, <br />Stipulation No. 6, and Stipulation No. 11. All previous <br />stipulations had been satisfied or incorporated into the permit <br />application package prior to issuance of the renewal permit. No <br />additional stipulations have been placed on the permit since the <br />permit was renewed. <br />Stipulation No. 6 <br />Stipulation No. 6 was originally placed on the permit in March, <br />1983, and specifies the information to be included in an annual <br />hydrologic report to be submitted to the Division by December 1 of <br />each year. This is an ongoing stipulation. A review of the files <br />indicates that the operator has consistently met the time frame and <br />informational requirements specified in the stipulation. <br />Status: Active <br />Stipulation No. 11 <br />Stipulation No. 11 was added during the 1993 permit renewal, and <br />required resolution of certain issues raised during the renewal <br />review pertaining to: <br />1) Landowner notification regarding undermining <br />2) Upper diversion ditches at waste pile <br />3) Cross-section drawing of sediment pond <br />Item 1 was satisfied by a letter and certificate of service listing <br />property owners who were mailed the letter notifying them of mining <br />beneath their property and the possibility of surface subsidence. <br />The letter and certificate of service was received by the Division <br />August 23, 1993, and was deemed adequate to satisfy the <br />notification requirements of Rule 4.20.2. <br />Item 2 was satisfied by Technical Revision No. 11, approved March <br />24, 1994. <br />Item 3 was satisfied by Minor Revision No. 8, approved October 6, <br />1993. <br />Status: Complied with March 24, 1994 <br />5 <br />
