(1991 Supp.). Attached please find a copy of the application which <br />needs to be resubmitted to this office with an on final signature. The well permit <br />application must be returned to this office in a timely manner for processing. <br />Please note that the application must have an original signature, the receipt <br />number (315613A), and reference to this substitute water supply plan. No fee is <br />required with this plan. <br />2. The total surface area of the lake created after December 31, 1990 must not <br />exceed 24.3 acres and uses must not exceed 34.29 acre-feet of evaporative loss, <br />5.14 acre-feet of water in the product, 2.5 acre-feet for dust control and 0.70 acre- <br />feet for material washing. Should the total consumption at ttie pit exceed these <br />amounts prior to the expiration date of this plan, an amendment needs to be filed <br />with this office. <br />3. Approval of this plan is for the purposes stated herein. Any additional uses for <br />which the water may be used must first be approved by this office. <br />4. The replacement water which is the subject of this plan cannot be sold or leased <br />to any other entity during the term of this approval. The accounting is subject to <br />approval and modification by the Division Engineer. <br />5. Adequate accounting of diversion and replacement must be provided to the <br />Division Engineer and Water Commissioner on a monthly basis or other interval <br />acceptable to both of them. All amounts shall be requested in acre-feet. <br />
