.; <br />Section II -Revisions to the Permit <br />Since the permit issuance, the Division has approved the following revisions: <br />Permit Revision No. 04 was received on November 8, 1993. The revision added two leases to <br />expand the underground mining operations, changed the mining plan to include mining both the <br />upper and lowE;r seams of coal, and revised the general format of the permit. The revision was <br />approved on September 13, 1994. <br />Technical Revision No. OS was received on June 5, 1993. The revision changed the size and <br />configuration of the permanent waste pile french drain and was approved on July 21, 1993. <br />Minor Revision No. 11 was received on October 26, 1992. The revision allowed NKC to <br />remove two smaller office trailers and replace them with a larger trailer. The revision was <br />approved on October 27, 1992. <br />Minor Revision No. 12 was received on June 10, 1993. The revision updated the groundwater <br />monitoring information in the permit and was approved on July 2, 1993. <br />Minor Revision No. 13 was received on July 29, 1993. The revision updated Map C-4 to <br />include the topsoil borrow area and was approved on August 3, 1993. <br />Minor Revision No. 14 was received on September 7, 1993. The revision updated the drainage <br />control map and was approved on September 13, 1993. <br />Minor Revision No. IS was received on September 12, 1994. The revision was for the addition <br />of a diesel storage tank and was approved on September 13, 1994. <br />Section III -Status of Stipulations <br />All stipulations. from the original permit were resolved prior to the issuance of the permit renewal <br />in 1987. No stipulations were attached to the latest permit renewal or to Permit Revision <br />No. 04. <br />Section N -Decision to Require Revisions <br />As a result of this midterm review, the Division will require revision(s) of the permit to address <br />the following concerns with the currently approved King Coal Mine permit application. <br />1. DMG Application Form, pages 3 and 4. Please update the acreage amount to reflect <br />current conditions showing the total permitted acreage, affected acreage, and disturbed <br />acreage.
