General Ory_anization <br />1. The Division believes that the Table of Contents needs to be updated in order to provide <br />clarity to the permit application. Right now, the only Table of Contents is the application <br />requirements form. Amore complete, fotmal Table of Contents, which would include maps, <br />exhibits, plates, drawings, attachments, reports, etc., would provide better organization. <br />Rule 2.03.4 <br />2. Section 2.03.4(2) lists the address of Sunland Mining Corporation as being in Indiana. <br />However, the Division has been sending inspection reports to Sunland in Kentucky. Please <br />update the address, if applicable. Also, please update the name and address of the <br />applicant's registered agent, if applicable. <br />3. Please update the list of owners and controllers shown in Section 2.03.4(3) and the operations <br />listed in Attachment A, if applicable. Attachment A lists permit transfers that were pending <br />in 1992. <br />Rule 2.03.8 <br />4. Please update Section 2.03.8 on page 8 to include cessation of mining and the current status <br />of reclamation efforts. <br />Rule 2.05.3(3)(c) <br />5. On page 128.1 of the permit application, it is stated that there are only two roads used <br />exclusively by the mine operator. They are the two access roads to the mine portals. Please <br />add that there is an access road to the bathhouse. <br />Rule 2.05.4(2)(e) <br />6. On page 187.1 of the permit application, there is a letter from the landowner requesting that <br />Brome, Timothy and Orchard grass be used in the seed mix. However, on page 184, the <br />approved seed mix does not include Timothy or Orchard grass. Does the operator wish to <br />revise the approved seed mix? <br />Rule 2.05.4(2)(e) <br />7. On page 184.2 of the permit application, the mine portal sealing plan is described. This plan <br />involved the use of backfilling material with no use of a concrete block barrier. However, <br />reference is made to the use of concrete block barriers on page 187.2, "Mine Portal Sealing <br />Plan"; page 184.1, "Sealing of Openings"; and on page 190.3, "Explosion-Proof Seal." For <br />the sake of clarity, please either remove references to a concrete barrier seal or add a note to <br />each reference, stating that the final portal sealing did not involve a concrete barrier seal. <br />8. On page 184.2 of the permit, it is stated that the operator will send to this Division a copy of <br />the notification sent to MSHA stating that the mine portals have been sealed according to <br />3 <br />
