Phase Nc Suwmary No. 6 Page 3 043-2170 <br />Earthworks monitor performed eight nuclear moisture-density tests (SLNc-070 through SLNc-77) <br />and performed two nucleaz moisture-density retests on SLF that required reworking (SLNc-70a, and <br />75a) located within the Phase IVc OSA. The retested SLF met the requirements of the <br />specifications. Field test results are presented in Attachment B. <br />Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, and <br />destructive). Geomembrane placed in the Phase IVc OSA met the project specifications. <br />Golder visually observed the placement of DCF in the Phase IVc OSA area. Golder CQA monitors <br />(one monitor per dozer when dozers are greater than 50 feet apart) observed DCF placement and <br />verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic (i.e., Caterpillar <br />Model 740 articulated trucks). <br />Golder observed placement of HVSC piping in the Phase IVc OSA, verifying installation and layout <br />per the project plans. <br />Golder visually observed removal of DCF June 15 from the Phase IVc OSA area to allow for <br />inspection of the Phase IVc geomembrane and HVSC piping due to equipment damage. Golder <br />-- ~~ -marked the defect damage (24T) caused~to the geomembrane, then" repaired and non-destructively ~. - - <br />tested the defect by the end of this reporting period. <br />Laboratory Tests <br />Golder obtained a QC sample of bentonite-amended SLF (SLF-QC-13). Laboratory index testing <br />was conducted on samples of bentonite-amended SLF at either the Golder Soils Laboratory in <br />Lakewood, Colorado, or at the field trailer to verify suitability of materials prior to use in the field. A <br />summary of QC laboratory test results for SLF materials are available at the Golder field trailer. <br />Golder obtained QA samples of SLF (SLFc-QA-07) and DCF (DCFc-02) for laboratory testing. The <br />samples tested met the requirements of the specifications and results are presented in Attachment B. <br />Meetin¢s <br />CC&V held a Weekly Contractors' meeting on June 15, 2004. Items discussed included Safety, <br />Environmental, Schedule, QC documentation, and CQA.
