MR-21 proposed approval decision was issued June 24, 1994. The revision addressed revegetation <br /> specifications for the lower benches of CRDA-1 and CRDA-2. <br /> MR-22 proposed approval decision was issued June 20, 1994. The revision allowed for a temporary ramp <br /> access road from the Cameo Borrow Area No. 2 (CBA-2), onto the CRDA-I. The temporary road has since <br /> been reclaimed. <br /> MR-23 proposed approval with conditions was issued August 29, 1994. The revision updated ownership and <br /> control information. The condition attached was Stipulation 13, addresed in the previous section of this <br /> document. <br /> MR-24 was withdr4y4pa&&porno;*$1994. Changes eftinally proposed in MA-24 were resubmitted, wi* <br /> some changes, in AJ29. <br /> MR-25 proposed approval decision was issued January 13, 1995. The revision incorporated a rill and gully <br /> plan which includes the requirement for annual rill and gully inspection. Seeding time frames for drill seeding <br /> and hydro or broadcast seeding were also specified in the revision. <br /> MR-26 proposed approval decision was issued January 6, 1995. The revision allowed for addition of a <br /> concrete pad fuel island for surface storage tanks at the North Portal Area, and a baghouse for dust control at <br /> Conveyor Transfer No. 2. <br /> MR-27 proposed approval decision was issued Febmsary 2, 1995. The revision allowed for a 5000 gallon fuel <br /> tank within a dirt bermed spill containment area near the guard shack at the Roadside North Facilities Area. <br /> MR-28 proposed approval decision was issued May 23, 1997. The revision set forth a drilling plan for more <br /> detailed hydrologic characterization of the Rollins Sandstone within the permit area. <br /> MR-29 proposed approval decision was issued June 5, 1997. The revision allowed for installation of two <br /> double walled fire resistant fuel tanks in the South Portal disturbance area;one at the existing RSS fuel island <br /> and one in the vicinity of the preparation plant. <br /> SO-1 proposed approval decision was issued September 23, 1997, wherein Permit C-81-041 was transferred <br /> from Powderhom Coal Company,a Peabody Holding Commpany subsidiary, to Powderhom Coal Company, <br /> a subsidiary of Quaker Western Coal Company. One condition, Stipulation No. 15, was placed on the <br /> approval decision. The stipulation required that proof of resolution of six Kentucky Civil Penalty Violations <br /> associated with Permit No. 8670201 and three Virginia CMIS violations associated with Permit No. 2850, be <br /> submitted to the Division as soon as available. <br /> Permit Revision No. 2 Review Process <br /> Powderhom Coal Company(PCC)submitted an application for Permit Revision No. 2, on May 3, 1996. The <br /> revision application consisted of a six volume permit application package, which was to replace the existing <br /> 8 volume permit application package. The submittal responded to concerns which had been identified in the <br /> Division's mid-term permit review, and in addition the application was comprehensively reorganized to <br /> eliminate discrepancies and duplication,and to present required information in a more accessible format. <br /> 8 <br />
