The following adequacy issues resulted from a review of hydrology topics in
the Blue Ribbon permit application.
6. Please revise Exhibit H-1 to reflect the existing sediment control system
at the site. The Division's most recent copy of the map was certified in
June, 1986. It shows diversion ditches that no longer exist (B and C)
and ditches in different locations then exist at the site (D and E).
Also, include the drainage pipe on the north side of the bench on this
map.
7. The Division received a map certified September 16, 1988 entitled "Mine
Bench Slump Repair, Plan of Drain Locations." Please revise this map to
include the 1989 repair work and include it within the permit application
as an exhibit. Provide a reference to this map and the method of
installation within the narrative.
8. a. Is there an one map which accurately reflects the location of the
two surface water sampling stations? Page 124 indicates that they
are located on the north and south edges of the disturbance;
Exhibit P, revised January, 1982, scale 1" = 200', shows them 1400'
apart, another Exhibit P, dated March 10, 1982, scale 1" = 2000'
show them 3000' apart. Please accurately locate both stations on a
map.
b. Has the location of the southern surface water monitoring station
changed over time?
9. What is the status of the Hubbard Creek alluvial well which was used as a
source of water for mining? If this was abandoned, please submit
abandonment reports per Rule 4.07.3(3).
10. The staff was unable to find documentation of embankment specifications
for the three remaining ponds per Rules 4.05.6(8)(e) and (g). Please
reference the location of this material within the permit application or
provide appropriate design documentation.
-3-
