f. Insurance shall include provision providing notification to the State 10 days prior to <br />cancellation of the coverage. <br />Automobile Liability (Minimum). The contractor must carry a minimum of $600,000 combined <br />single limit. <br />VENDOR OFFSET <br />Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed to <br />state agencies under the vendor offset intercept system for: (a) unpaid child support debt or child <br />support arrearages; (b} unpaid balance of tax, accrued interest, or other charges specified in <br />Article 22, Title 39, C.R.S.; unpaid loans due to the student loan division of the Department of <br />Higher Education; (d) owed amounts required to be paid to the unemployment compensation fund <br />and (e) other unpaid debts owing to the State or any agency thereof, the amount of which is found <br />to be owing as a result of final agency determination or reduced to judgment as certified by the <br />controller. <br />Work Description <br />The attached General Conditions for Purchase Orders and Purchase Order Terms and Conditions <br />apply to all work. . <br />Project Overview <br />The goal of this Project is to safeguard public health and welfare by accomplishing stabilization of <br />a collapsing cribbing wall, and using the material behind the wall to close an abandoned <br />underground mine, which is open. Generally, mine waste materials located behind and above the <br />cribbing will be excavated, and removed to a disposal site located below the adjacent road. The <br />mine waste material will be deposited in a manner which will prevent access to the underground <br />mine opening. Non-coal earthen materials located at and adjacent to the disposal site will be used <br />as a cover /topsoil for the mine waste. The excavation area and the disposal area will be <br />revegetated. <br />NOTE that volumes, distances and areas stated in this document are ESTIMATIONS ONLY. <br />CONTRACTORS are responsible for verifying all estimations prior to bid submittal. <br />WORK TASKS <br />The tasks included in this Project are as follows: <br />Task 1: Coal Mine Waste Excavation <br />Task 2: Topsoil Salvage and Application <br />Task 3: Final Grading <br />Task 4: Revegetation <br />Task 5: Equipment Rental <br />Task 6: Scrap Removal <br />
