<br />-4- <br />Further, National King Coal must prepare an appropriate <br />geotechnical and stability analysis of the proposed backfilled <br />configurations for the portal bench areas at the King Coal Mine. <br />These analyses shall utilize accepted slope stability mathematical <br />analysis techniques, material strength determinations and <br />appropriately detailed geotechnical knowledge of the site, in <br />order to complete the required stability demonstration. A report <br />of this required stability analysis and the appropriate <br />geotechnical investigations or other information acceptable to the <br />Division demonstrating stability, shall be submitted to the <br />Division for approval within 120 days following issuance of the <br />approved permit. <br />National King Coal has provided the Division with an acceptable <br />mass balance demonstration and geotechnical and stability analysis. <br />II. Banding <br />The Division has reviewed both the amount and conditions of the bond for the <br />King Coal Mine in accordance with Rule 3.02.2(4) and has found the bond to be <br />acceptable. <br />III. Summary <br />As a summary of this review, the Division has identified the following items <br />of concern. <br />1) A revised table of contents must be submitted for the permit <br />application which includes the appropriate references to and <br />locations of all additional or revised material, exhibits, maps <br />and stipulation responses. <br />2) The permit application form must be updated to show the current <br />contact person and any other revisions. <br />3) A current insurance certificate must be submitted to the Division. <br />4) Evidence of renewal of the NPDES discharge permit must <br />be submitted to the Division. <br />5) A map showing the "Huntington" irrigation ditch in relation to the <br />underground workings must be submitted to the Division. <br />6) The alluvial valley wells' water level must be reported along with <br />each quarter's water quality data. <br />Items 1 through 5 must be submitted to the Division as minor revisions by <br />December 31, 1984. Item 6, the required water level monitoring information, <br />can be submitted as part of each quarterly water monitoring report. <br />Doc. No. 5056E <br />
