Permit Policv for Minine Activities: (Continued) <br />(3.) violate the WQS of the receiving stream segment(s) and <br />beneficial uses (for active mining facilities). <br />4. The district engineer from the WQCD has visited the site and <br />concurs with inactivation. DE concurrence shall be <br />specifically stated on the appropriate inactivation request <br />form. <br />5. Upon request for permit inactivation (entity submittal of the <br />signed inactivation request form), the permittee shall be <br />provided a letter from the P 6 E Section within 30 days which <br />'outlines the conditions for the demonstration. <br />Failure to satisfy any of the above will be grounds for continuing <br />to require a permit for the site. For further information and <br />details regarding this policy, the permittee should request a copy <br />of this WPQ-S policy. <br />C. Monitoring and Reporting <br />1.
