The applicant shall submit a Flood Hazard Development Permit for conducting mining activities within the 100-year floodplain which alter the course of the river within 90 days of authorization to alter said river course being granted by the requisite Federal and State agencies with oversight. 4. The applicant shall file an updated Mined Land Wildlife Statement/Evaluation reflecting the proposed reclamation for the additional acreage being received from the State. Said Statement/Evaluation shall be submitted to the Department of Planning and Development within sixty days of the closing and consumation of the Land Exchange Contract between Valco, Inc., or their assigns or heirs, and the State of Colorado or their assigns or heirs. 5. The applicant shall file a copy of their Annual Mined Land Reclamation Report with the Department of Planning and Development at the time it is submitted to the State of Colorado, Department of Natural Resources, Mined Land Reclamation Division. 6. Special Use Permit No. 479 (Amended) shall be placed on a five-year review (July, 2000) or be reviewed as the property description for the mining operation requires amendment.
