t <br />Waste Prep~ation, Manijestixg, and Sleipment <br />In October 1995, personnel from USPCI Laidlaw and WALSH prepared the Waste Profile Sheet <br />required by USPCI for PCB waste shipments. The four drums were accepted for off-site <br />incineration on October 23, 1995. On November 2, 1995 the four drums were again inspected <br />for structural weakness, labeled, properly manifested, and loaded onto a USPCI truck for transport <br />to USPC[ Laidlaw's disposal facility in Grassy Mountain, Utah. One drum required the use of <br />an overpack drum for proper shipment. Copies of the waste profile sheet and notification of <br />waste acceptance are attached to this report. Copies of the transportation manifests were given <br />to the CDMG at the time of shipment, and copies of the certificates of disposal were sent directly <br />to the CDMG. <br />Conclusions <br />A total of four 55-gallon drums were chazacterized, labeled, properly manifested, and transported <br />by USPCI to USPCI Laidlaw's disposal facility in Grassy Mountain, Utah. The material in the <br />drums was incinerated at the facility and certificates of destruction were sent directly to the <br />CDMG. Wastes were handled per the regulations governing their packaging, transportation, <br />treatment, and destruction. These regulations include the Toxic Substance Control Act, Resource <br />Conservation and Recovery Act, U.S. Department of Transportation regulations, and the <br />Occupational Safety and Heath Act. <br />Limitations <br />The acceptance of waste by the receiving facility was based on information obtained from the <br />generator (CDMG), on laboratory analysis, and on field observations. Therefore, the sources of <br />information to perform this work include documents, oral statements, and other information from <br />parties outside of WALSH's control. The accuracy of the information cannot be guaranteed by <br />WALSH. <br />WALSH appreciates the opportunity to work with the CDMG on this project.
